Healthcare CFO Services: Why December’s Site-of-Care Change Rewrites Surgical Cash Planning

Published: Tuesday, October 13, 2026, at 3:00 PM ET

Healthcare CFO Services: Why December's Site-of-Care Change Rewrites Surgical Cash Planning is a cash forecast problem, not a coding problem. Blue Cross and Blue Shield of North Carolina (Blue Cross NC) posted a new surgical site-of-care policy on October 1, effective December 1, 2026, for Medicare Advantage. The reimbursement notification confirms the date, and Commercial, Inter-Plan Program and Federal Employee Program business is unaffected.

What changes December 1 for Medicare Advantage facility claims

Facility outpatient surgical services performed in an ambulatory surgery center (ASC) continue at 100% of the Blue Cross NC allowance. Hospital outpatient department (HOPD) facility claims reimburse at 100% only when the site-of-care criteria are met and Modifier KX is reported. When criteria are not met, the policy directs that KX not be reported and pays 60% of the allowance.

A 40-percentage-point swing on an affected claim is a cash event. It is not automatically a 40% loss of profit: account for the procedure's cost, eligibility, capacity, staffing, and the operating expense of a different site.

Healthcare CFO Services: model the exposure before December 1

Separate Medicare Advantage facility claims from other payer categories. For each affected procedure, identify the planned setting, expected allowance, eligibility, and documentation ownership, then model HOPD claims twice: criteria met with correct KX reporting, and criteria not met at 60%.

✅ Reconcile surgical schedules with payer and facility-claim data.
✅ Assign accountability for confirming site-of-care criteria.
✅ Pre-bill review for accurate KX reporting.
✅ Track December and January remittances separately.

This is cash position analysis turning a policy update into forward looking financing decisions. The goal is not to move every case; it is to preserve patient access while managing liquidity with certainty.

The questions practice leaders are asking

“We already document medical necessity, so why would the same surgery pay less in December?” Because the policy ties HOPD facility reimbursement to its own criteria and reporting requirements. Existing documentation should be tested against the policy, not assumed to satisfy it.

“Should we shift cases to an ASC before December 1?” Only if clinical appropriateness, ASC capacity, equipment, anesthesia, and case contribution margin support it.

Funding the timing gap with AR-Backed Working Capital

Accounting is history. Financing is forward looking. Where fixed costs land before collections, AR-Backed Working Capital is a planning option. Under White Coat Financial Partners' non-notification model, the practice keeps its own billing and collections; we do not take possession of receivables, contact patients, or intervene in billing. Lending is against the aggregate sum of monies due across total receivables, not individual claims, is AR-backed rather than based solely on personal credit scores, and scales from solo practices to multi-location groups. Repayment follows cash flow on a clear schedule matching how the practice actually gets paid, with no compounding interest.

Equipment leasing complements surgical and diagnostic technology planning. A lease pays for use rather than ownership; leases with purchase options support upgrades to newer imaging and diagnostic technology at term's end. Lessees may deduct the full lease payment as a business expense in the year of payment (check with your tax advisor to see if you qualify).

FAQ: December’s surgical site-of-care change

When does the Blue Cross NC policy take effect?

The policy is effective December 1, 2026, for applicable Medicare Advantage outpatient surgical facility claims.

What happens to eligible HOPD and ASC facility claims?

ASC claims continue at 100% of the Blue Cross NC allowance. Eligible HOPD facility claims are reimbursed at 100% when the criteria are met and Modifier KX is reported. HOPD claims that do not meet the criteria are reimbursed at 60%; KX should not be reported for those claims.

Does this policy apply to Commercial, IPP, or FEP claims?

No. Blue Cross NC states that the policy applies to Medicare Advantage and does not apply to Commercial, Inter-Plan Program, or Federal Employee Program business.
